This booklet supports collaborative goal setting with the person that you are working with.
Introducing the role of the occupational therapist to the person, this tool encourages the person to identify what is important to them and why? How confident they feel when participating in the occupations that they need and want to do. It also looks at the person's daily routine and encourages conversation about what the person feels their strengths and limitations are.
It then encourages the person to consider what they want to change, supports them to write therapeutic goals to achieve this change and write a plan of action.
